Sažetak
With the opening of the new Department for forensic psychiatry, new organizational and technical possibilities for the treatment of persons not guilty by reason of insanity (ngri) have appeared. so far, these patients were treated in inadequate conditions as all of the 70 patients were placed together in one ward and there were no options to group them into smaller heterogeneous (diagnostically and according to risk assessment) groups. Due to these infrastructural limitations, we were unable to group patients based on whether they were allowed to leave the ward (and go to the hospital facilities outside the ward) or not. there will be four wards in the new building ; patients will be placed on different wards according to their diagnoses and risk levels, and different psychotherapeutic and sociotherapeutic possibilities will be implemented. there will also be one ward with eight beds, and patients with a high risk of auto- and heteroaggression as well as those who are not able to establish abstinence from psychoactive substances (because in the old ward it was not possible to completely prevent them from getting these substances) will be placed at this separate unit with higher levels of care and security ; at the same time, other patients with a lower security risks will not be limited in their sociotherapeutic activities and in their ability to leave the ward. as the same staff (with one psychiatrist and a few nurses more) and the same procedures (pharmacotherapy, psychotherapy, sociotherapy) will be implemented, these changes will represent somewhat of a natural experiment, and we will be able to monitor the influences of infrastructural changes on the mental state and forensic risk of patients not guilty by reason of insanity.
