Sažetak
Objectives: In this paper we present the importance of a complete person-centred approach that includes somatic examination to establish an accurate diagnosis in young men with emotional and family issues. Methods: Since the childhood period is considered to be necessarily related to a sense of care freeness and health in children and adolescents the existence of serious illness and disorders is often neglected. Therefore, in our paper we wish to emphasize a holistic approach to the person which stresses the importance of involving somatic medical practitioners in order to establish a proper diagnosis to aid the adolescent. Findings: A 16-year-old adolescent was sent to a child psychiatrist as an outpatient with pronounced emotional difficulties. During the psychiatric treatment the patient complained of frequent headaches, feelings of weakness, vomiting, lack of concentration, mood oscillations, sleeping disturbances and fainting. His family history showed disrupted relations manifested through parental divorce, father's ilness and brother’s psychiatric treatment due to depression and a suicide attempt. Our patient was further referred for neuropaediatric treatment due to additional physical symptoms. The neuropediatric examination and magnetic resonance imaging of the brain verified a benign tumour of the pineal gland. Diagnosis of the tumour additionally affected the patient's already weakened psychological state. Conclusion: Irrespective of the pre existence of emotional and family difficulties we cannot automatically presume the existence of psychiatric disorders in children and adolescence referred for treatment. In every case a holistic approach must be adopted in order to rule out any somatic causes of apparent psychiatric difficulties. This is of up most importance since children and adolescence represent a vulnerable population in our society.
