Ackground Nonadherence to medication is a growing concern to healthcare systems, physicians and other stakeholders because of mounting evidence that it is prevalent and associated with adverse outcomes and higher costs. Self-report scales have the benefits of being cheap and easy to administer. The objective of the paper is to check the basic metric characteristics of the Culig scale, in an alternative way. Methods Design: The cross- sectional study Setting: 223 patients in Zagreb pharmacies Main outcome measures: Cronbach's coefficient valid for results formed as a projection of the results of each of the respondents to the first main object of measurement questionnaire, which is defined as the factor score on the first principal component intercorrelation matrix. Statistical analysis: Student’s t-test and Chi- square test with a significance level of p < =0.05 were used when appropriate for the evaluation of the results. Results Culig self-questionnaire was administered to a sample of 223 respondents, with 59 indicators, and reliability coefficient α in this measurement is 0.92, what is essential higher than in a conventional way (0.89). Conclusions There are many self-report scales for measuring medication adherence and their derivatives (or subscales). The used alternative processing methods of individual results by Culig questionnaire and some psychometric properties are defined. The results in this way are significantly better than those in certain conventional way of processing. The 16-item Culig scale with very good internal consistency reliability (Cronbach's alpha = 0.89) has been inaugurated in Croatia. Such analysis opens the possibility of multivariate analyses of the results, what is important for further research.
