Sažetak
Obesity is one of the leading health problems and an important factor in development of cardiovascular diseases. There is a clear pathophysiological and clinical link between obesity and a series of metabolic disorders. Metabolically healthy obese people are separate subgroup of obese patients. Those patients have increased body mass, although without clear metabolic disturbances. There is no consensus of metabolic health description in obese patients. The same parameters are used as for the definition of metabolic syndrome (total, HDL and LDL cholesterol, triglyceride, glycaemic nasal discharge, blood pressure, insulin resistance index, inflammatory parameters like high- sensitivity CRP). Those parameters different authors apply in a variety of combinations, i.e. some include insulin resistance index and inflammatory parameters, some not. However, for patients with increased body weight, the waist circumference in definition of metabolic health is not taken into account. According to current researches their prevalence in European countries ranges from 11 to 26%. The question of the highest priority is what will happen in the future with metabolically healthy obese patients ; do they carry higher cardiovascular risk or not. Until now long-term follow-ups found that at least half of them over the years develop some of the metabolic disorders. Compared to healthy patients with normal body weight they carry a higher cardiovascular risk, though not as high as obese patients with already present metabolic disturbances. In a large number of metabolically healthy obese patients the current condition of metabolic health is a part of transition from health to disease-we cannot conclude that their metabolic health will last forever and that they are going to be without risk of development of atherosclerosis permanently. It is important to monitor them, evaluate their overall cardiovascular risk and finally, if necessary, to treat them. In short, metabolically healthy obesity is not benign condition and it is mandatory to promote weight loss in all aspects of life.
