Sažetak
A potential role of adipocitokines originating from visceral adipose tissue: adiponectin, leptin, resistin and dipepetidyl peptidase-4 (DPP-4) with the endothelial function parameters: NO (nitric oxide), von Wilebrand factor (vWF), reactive hyperemia index (RHI) and the pulse wave velocity (PWV) in the development of microvascular complications in patients with type 1 (T1DM) and type 2 diabetes (T2DM) are investigating in the “Relationship between endothelial dysfunction driven by adipocitokines and the development and progression of microvascular complications in patients with type 1 and type 2 diabetes” (ADIEDM) prospective study. At baseline, T2DM have lower adiponectin level (5.2 vs 10.6 ug/ml, p<0.001), DPP-4 activity (38.3 vs 45.4 U/L, p<0.001), and RHI (1.7 vs 1.8, p=0.02), and higher leptin level (19.1 vs 9.7 ng/ml, p<0.001), compared to T1DM. Multiple regression analysis showed that vWF is associated with increased serum leptin (p<0.001, r=0.27) in T2DM and with increased PWV in T1DM (p=0.01, r=0.28), while NO is associated with decreased RHI in all patients (p=0.002, r=-0.16). In patients with hemoglobin A1c < 7%, NO is associated with increased serum adiponectin (p=0.01, r=0.21). Effects of adipocitokines on endothelial function parameters and incidence and progression of microvascular complications in type 1 and type 2 diabetes should be clarified after 4 years of follow up.
