Sažetak
The field of psychotraumatology has been developing intensively for the last forty years, but there are still many controversies related to etiology, classification, clinical picture, neurophysiological events underlying clinical manifestations and of course treatment approach. The posttraumatic reaction and disorder spectrum still does not cover all the clinical syndromes we meet on a daily basis, which results in difficulties both in the research and in the legal administration. The existing classification diagnostic framework of both leading classification systems in different ways tries to solve these deficiencies but endeavoring to purely descriptive approach significantly reduces all the wealth of post-traumatic presentation and leads to simplification that does not contribute to a better understanding, on the contrary. For the last 20 years, especially in Herman and van der Kolk's works, the need to distinguish between posttraumatic stress disorder and syndrome beyond PTSD has been emphasized, which we see in people with long- lasting posttraumatic disorders. There are different suggestions for the name and for symptom groups of that syndrome. This problem is only partially solved by introducing a diagnostic category of Enduring personality changes due to catastrophic experience . In fact, this diagnostic category has led to an additional series of controversies. New diagnostic category- Complex PTSD- that has been accepted for ICD-11 rises additional dilemma and controversies. The presentation will show possible areas of research on the diagnostic plan with emphasis on practical difficulties arising from the undefined classification status.
