Sažetak
We continue with our work to evaluate retrospectively the use of video-assisted thoracis surgery (VATS) in the treatment of the traumatic hemothorax and posttraumatic empyema copared with closed tube thoracostomy. We based our work on the review of the results of VATS treatment in 38 patients with hemothorax after blunt trauma in the period from Semptember 1994. to Semptember 1999. and the closed tube thoracostomy treatment in 70 patients in the period from September 1990. to September 1999. Median age in teh VATS group was 48 (18-70) years. In this group one patient (2, 6%) developed empyema and chest tube drainage was left in placefor 3, 9 (3-7) days. Median age in the group tretaed with close tube thoracostomy was 50 (20-74) years. In this group 11 paients (15, 7%) developed empyema after traumatic hemothorax in 12 patients and the chest tube drainage was left for 6 (3-18) days. Also we used VATS in debridment and irrigation for posttraumatic empyema after traumtic hemothorax in 12 patients. That was successful in all cases. The difference in groups concerning the percentage of empyema and duration of chest tube drainage is statistically significant (p<0, 01). VATS treatment was safe and effective operative strategy for the treatment of traumtic hemothorax and posttraumatic empyema and we recommend it.
