Sažetak
Introduction: Twenty five years of experience in orthodontic treatment of cleft lip and palate with reverse overjet (CLPRO) in our country pointed out at the problem of treatment of CLPRO in early mixed dentition. So far, in this phase of dentition, orthodontic therapy was not conducted or removable orthodontic appliances, lingual arch, or facial mask(Delaire type) wwere used. The first disturbed the normal function of the tongue, while the third was accepted with difficulty by children.Theerefore, the rigid protrusion labial arch (RPLA) was devised. Objectives: To consider a approach in the treatment of CLPRO by RPLA and analyse its effect by graphoanalytical method. Method: RPLA is made of wire 1.00 mm in diameter and it is fixed on the buccal tubes on bands wich are on the maxillary first permanent molars. there are also bands on the maxillary central permanent incisorss, and the arch is shifted from them. The connection between the incisors and the arch and activation of biomechanical activity is realised by elastic chain between the bands and the arch wire in accordance with graphoanalytical analysis. Results: Clinical application of this arch during 2 years and its biomechanical activity, analysed by graphoanalytical method, sshowed advantages with the treatment of the aforementioned anomaly. The devising of the device is simple and it is easily applied. Conclusion: In orthodontic treatment of CLPRO in early mixed dentition, RPLA showed advantages over other orthodontic appliances. When regular overbite of incisors is achieved, orthodontic treatment continues with one of the fixed techniques we are using.
