Sažetak
A 76-year old woman was hospitalized at our Department in January 1991 for butterfly shaped, brownish-black extensive centrofacial crusting which caused painful and burning sensation. According to history data, in 1986 she was treated with electrocoagulation because of histological proved basal cell carcinoma of the upper lip. One year later, a small erosion appeared at the same localization and healed spontaneously. During 1990, sharply delineated reddened patches on the nose and cheeks were observed. Unfortunately, she had no medical documentation about the previous diagnostic procedure and treatment. Only according to personal history data, she was at first treated with some intralesional injections, probably corticosteroids. Then she started with local application of 5-fluorouracil, Efudix cream. The above described centrofacial crust formation appeared during 3 to 4 month local application of 5-FU, twice daily because of suspect carcinoma of the skin, as patient said. Pathohistologic analyses of two biopsy specimens from the cheeks did not confirm either the above mentioned diagnosis, any of differential diagnostic skin diseases, too. She was only treated locally with antiphlogistics for three weeks. The result was complete regression of skin changes, suggesting that it was an extensive local skin reaction to the prolonged and uncontrolled application of the local cytotoxic drug 5-fluorouracil.
