Sažetak
Heart Failure (HF) is growing public health problem, and Metabolic Syndrome (MetS) is major concomitance in Acute HF (AHF) associated with increased HF risk. The purpose of present research was to examine the impact of MetS in hospitalized AHF patients (pts) on outcome defined as the length of hospitalization, in- hospital and 3-months follow-up mortality.The study was performed as prospective, non- interventional, single-center observational study including hospitalized AHF pts recruited from the Emergency Department (ED) in the University Hospital Centre Sisters of Charity (UHC SC), Zagreb, Croatia from November 2013 to February 2015.Participants were divided into two main groups depending on the presence of MetS and compared according to baseline characteristics, comorbidities, lipid levels, the length of hospitalization and mortality. Study analysis included data for 152 hospitalized AHF pts. 55.9% of investigated pts had MetS, equally by gender. Pts with concomitant MetS had a greater occurrence of relevant comorbidities. Further, triglyceride levels were higher in MetS group (P=0.001), and HDL level was lower in MetS group (P=0.004). MetS pts had better in-hospital (P=0.046), and 3-month follow-up survival (P=0.0072), although their length of hospitalization was longer (P=0.02). Results indicated that Reverse epidemiology paradox and controversy Obesity paradox emerged in our study. It was shown that lower BMI was independently associated with increased short-term, but not with 3-month follow-up mortality in patients with AHF. Patients with AHF should be carefully assessed for concomitant metabolic disorders and dyslipidaemias.
