From May 1993. to April 1997. laparoscopic cholecystectomy was accomplished in 1523 of 1621 (93, 4%) patients with cholelithiasis. There were 1375 (84, 3%) patient with chronic cholecystitis and 256 (15, 7%) with acute cholecystitis. Conversio were primarly due to marked inflammation, uncontrollable bleeding and techical difficulties. The mean hospital stay was 3 days (range 1-16). The patients were operated under general anaesthesia. In 15 patients we performed the operation under high peridural anaesthesia. There was no procedure related mortality. There were no common bille duct injuries. Pulmonary embolism occured in 8 patients without mortality. Postoperative complications occured in 50 patients (3, 1%). In conclusion, laparoscopic cholecystectomy is the procedure of choice for patients with cholelithiasis. The advantages over open cholecystectomy are: shorter hospital stay, less pain and earlier return to normal acitvities.
