Sažetak
Background:Unilateral spastic cerebral palsy (US- CP) is the unilateral motor impairment, congenital or early acquired, isolated or associated with additional impairments. Brain MRI is very useful tool to identify lesions causing US-CP and presence of associated disabilities. The aim of the study was to demonstrate the structural and functional deficits of US-CP. Method: The cross- sectional study which included children born in Zadar County in the period from 2003. to 2010, involved in "C28 RCP-HR" Register of cerebral palsy of the Surveilance of Cerebral Palsy Europe (SCPE). In 38 of 13 579 cerebral palsy was diagnosed by making cumulative prevalence of 2.79‰. Functional classification of CP and MRI findings was used according to SCPE. Furthermore, 33/38 (87%) were diagnosed with spastic disorders, of which 12 children (36%) had US-CP, five (41.7%) right-sided, seven (58.3%) left-sided. Results: We found predominantly periventricular white matter lesions in 7/12 children. The involvement of additional structures in this group such as the basal ganglia was found only in one child. The remaining five children had predominantly damage of the gray matter. Interestingly, in 8/12 children (67%) were found bilateral structural damage on brain MRI, despite the fact that they had clinically clear unilateral spastic CP without functional disability on the contralateral side. All children with predominant gray matter brain damage have a higher degree of language disorders and abnormal electroencephalogram. Two children with middle cerebral artery infarction had the third degree of expressive speech disorder on the Viking scale. Two children with predominant damage of the gray matter had severe epilepsy. Conclusion: Brain MRI may help to understand morphological background of the motor impairment in children with US-CP, but the process of plasticity, brain maturation and early intervention can also contribute to the final outcome and the degree of functional deficit.
