Sažetak
AIM: To analyze associations between chronic graft versus host disease (cGVHD) of joint/fascia with clinical findings, biomarkers, and measures of functional capacity. METHODS: Patients were diagnosed with cGVHD based on multidisciplinary assessment and the NIH Consensus Criteria. Evaluations included physical exam, laboratory studies, and functional measures including the 2- minute walk test and hand grip strength. Logistical regression analysis explored the relationship between these evaluations and the severity of joint and fascial cGVHD. RESULTS: Of twenty- nine patients, twelve patients (41.3%) manifested cGVHD of joint/fascia according to NIH Consensus Criteria. There was a significant positive correlation of joint/fascial cGVHD and skin cGVHD (rho= 0.712 ; P < ; 0.001), serum C3 complement level (rho= 0.405 ; P= 0.045), and leukocytes (rho= 0.406 ; P= 0.032). There was a negative correlation between 2-minute walk test (rho= -0.444 ; P=0.16), between per cent of cytotoxic T cells (rho = -0.456 ; P= 0.022), serum albumin (rho= -0.402 ; P= 0.047), and Karnofsky Performance Status (rho= -0.754 ; P < ; 0. 001). Binary logistic regression model found that cGVHD skin involvement is significant predictor (OR = 7.79 ; 95 CI : 1.87 - 32.56 ; P=0, 005) for cGVHD of joint/fascia CONCLUSION: Joint/fascia cGVHD manifestations are significantly associated with multiple biomarkers. Clinical measures that are predictive include worse scores in the 2-minute walk test, skin cGVHD, and a lower Karnofsky score.
