Key component in preventing the spread of infectious diseases, including foodborne diseases, is good hand hygiene. Unsatisfactory food handlers' hand hygiene is very important contributor to foodborne outbreaks. The aim of this study was to evaluate microbiological cleanliness of food handlers' hands in Dubrovnik area between 2010 and 2015. Microbiological cleanliness of hands was determined by swab or contact slide method after usual hand washing routine. Aerobic colony count and Enterobacteriaceae were examined in swabs/contact slides according to Croatian standards for microbiological cleanliness. 14.744 food handlers' hands swabs/contact slides were examined between 2010 and 2015. Of all 14.744 swab/contact slide samples 3.955 (26, 8%) were unsatisfactory. The unsatisfactory results of swabs/contact slides on anual basis were as follows: in 2010 - 33% (927 of 2.812) ; in 2011 - 31% (826 of 2.676) ; in 2012 - 28% (722 of 2.588) ; in 2013 - 24% (569 of 2.337) ; in 2014. 21% (462 of 2.199) ; in 2015. 21% (449 of 2.132). According to statistical analysis by χ2-test, there is statistically significant difference between 2010 and 2015 (p<0, 05) with downward trend of unsatisfactory results. Aerobic colony count has been the most frequent unsatisfactory parameter ; found in 3.923 swabs/contact slides (99% of unsatisfactory samples). Unsatisfactory level of Enterobacteriaceae has been found in 1.066 swabs/contact slides (27% of unsatisfactory samples). 1.029 swabs/contact slides (26% of unsatisfactory samples) had unsatisfactory level of both aerobic colony count and Enterobacteriaceae. We have also compared the results in this observed period between various food handlers. The unsatisfactory results were observed in 23% of cooks, 33% of waiters, 30% of confectioners, 27% of bakers, 20% of food sellers, 25% of butchers and 27% of other supporting staff. The best hand hygiene was observed in food sellers, and the poorest in waiters and confectioners. There is a significant improvement trend in food handlers' hand hygiene. We believe it is due to implemented HACCP system and good hygiene practice. Swabs/contact slides of food handlers' hands are the most frequent unsatisfactory samples – they made up 58% of all unsatisfactory swab/contact slide sampled at food premises. Hand hygiene in food handling is one of the key components for it's safety. Increasing the awareness and constant education on hand washing is neccessary in every HACCP system and good hygiene practice.
