Background: whereas some studies have suggested an association between the presence of lupus anticoagulant (LA) antibodies and women infertility, others have not confirmed these observations. Experts disagree regarding the relationship between LA and infertility and according to the present evidence-based data, this association remains controversial and inconclusive. However, it is obvious from our daily laboratory practice that testing for LA antibodies has become increasingly common in population of infertile women in the recent several years. The aim of this study was to analyze the results of LA evaluation in population of infertile women referred for testing at our laboratory during a three consecutive years (2012 to 2014). Methods LA antibodies were evaluated by a panel of commercial coagulation assays (Siemens, Germany) including three screening tests: activated partial thromboplastin time (APTT), APTT mixing test, dilute Russells viper venom time (dRVVT) screen or LA1 test and one confirmation test (dRVVT or LA2 confirmation test). Results The overall prevalences of positive LA results in unselected consecutive patients referred for testing were: 44/1025 (4.3%) in 2012, 46/873 (5.3%) in 2013 and 22/634 (3.5%) in 2014. Among all requests for LA testing, infertile women accounted for 13% (133/1025) in 2012, 13.3% (116/873) in 2013 and 12% (76/634) in 2014. Positive LA results in population of infertile women were found in two cases i.e., 1/133 (0.75%) in 2012 and 1/116 (0.86%) in 2013. None of the 76 infertile women referred for testing in 2014 had positive LA test result. Conclusion Laboratory investigation of LA antibodies among unselected consecutive patients has shown prevalence of 3.5-5.3%, which is consistent with the literature data. Infertile women referred for LA testing accounted for about 13% of all patients investigated. The prevalence of positive LA results in population of infertile women proved to be too low to justify nondiscriminate and routine testing in unselected population of all infertile women. The results support the present evidence that an association between LA antibodies and women infertility is a matter of controversy that should be carefully reviewed. These findings are of potential benefit from a health economics viewpoint in eliminating unnecessary investigations and their associated cost.
