Sažetak
Background: The potential advantages of immediate implant placement after tooth extraction are reduction of treatment time and surgical interventions, preservation of alveolar bone and soft tissue aesthetics. Many times immediate placement in fresh extraction socket can be more challenging compared to placement of implants in healed sites. Aim: The aim of this study was to evaluate and compare the success rate of immediate implant placement in fresh extraction socket compared to implant placement in healed site. Materials and Methods: 24 Dental implants were placed in 12 patients in the period March - April 2014. Each patient received 1 dental implant in healed bone and 1 implant in post extraction socket. After atraumatic tooth extraction implants were placed only in sockets with intact bucal/labial plates, 2 mm subcrestal without any grafting. For implants placed in healed sites the standard protocol from the manufacturer was followed. Periosteal release incision was made after implantation to achieve primary closure of the sites. Patients were followed-up for a period of 18 months (14 months after prosthodontic restoration). Clinical and radiographic evaluations were made to evaluate and determine if there was any difference between the groups. Results: No clinical signs of post operative infection were observed in any of the groups. All the implants in both groups were successfully integrated. At 18 months the mean marginal bone resorption was 0.50 mm for the implants placed in fresh extraction socket and 0.46 mm for the implants placed in healed sites. There was no statistically significant difference between groups. Conclusions: Within the limitations of this study it can be concluded that immediate placement of dental implants in extraction sockets is a predictable and successful treatment with no difference in comparison with implants placed in healed sites.
