Sažetak
The report deals with two cases of clinically proved multiple sclerosis, that in the initial course of their illness were erroneously mistaken for cerebral tumor cases: one for the brain stem and the other for the temporal lobe tumor. The basic clinical characteristics of both cases of pseudotumoral form of MS were: clearly located, single, large and fast progressive demyelinating lesion followed by focal neurological deficits (tetraparesis, speech disturbances, partial epilepsy). MRI presentation was: solid or ring-shaped formation with discret mass effect. Misled by rare occurence of this type of the disease, neurologists, neurosurgeons and radiologists misdiagnosed both cases-one as an inoperable brain stem tumor, and in the other one a temporal lobectomy was performed. In both cases the needed additional diagnostic work-up was not performed-primarily the CSL analysis with IgG quotient measurement and IgG oligoclonal band assesment. Our report strongly suggest that the diagnosis of multiple sclerosis is primarily the clinical one, demanding the accurate judgement of clinical and laboratory data in each case where the suspicion to the diagnosis of MS can be raised. Neuroimaging techniques cannot always be clearly distinctive in delineating CNS-tumors from the pseudotumoral form of MS. So, the most important factor in the accurate diagnosis of this form of MS still remains the clinical judgment of the clinician.
