Backgroaund There is a growing prevalence among asthma and chronic obstructive pulmonary disease (COPD) patients in Croatia and worldwide. COPD causes the quarter of world's mortality. Metered Assessed dose inhalers (MDI) and dry powder inhalers (DPI) are the most common drug application form. The drug does not reach lower bronchial branches, yet it is retained within the oral cavity and upper bronchial branches. That is the reason why the patients' education is the key action. Objective The purpose of this study is to evaluate patients' skill to apply the therapy using the MDI as well as s, who did, and for how long, conduct the patients' education. Methods There were 60 patients evaluated in this cross- sectional study from November 2014 to March 2015. Half of the examinees were members of the 'Jedra' association (Group 1). The other half of the examinees were dispnoic dyspnoeic patients on MDIs who called Department of Emergency Medicine within 10 days period ( February- March 2015.) (Group 2). There was a structured interview conducted with the the authors' questionnaire self-designed by authors themselves. Results The given results show that MDIs are used by 63% patients and DPIs by 35% patients. Inhaler technique has been useed by the patients between one month and 25 years. Patients' education is usually carried out in hospitals and done by the specialists (40%), nurses (10%), or both (12%). Prehospital patients' education is conducted by the general practitioners (10%). For the majority of patients (68%) education was carried out only once with the statistically significant difference (Group 1- 50%, Group 2- 87%, P=0.005). It was a single education with most patients (68%), with the statistic significance (P=0.005) within the evaluated group of patients. Only 18% of patients had two or more educations where there was also a statistically significant difference found (Group 1- 33%, Group 2- 3%, P=0.007)statistical signiffice (P=0.007) among these two groups. There was a difference between groups in MDI appliance for the 'Mouth arround the device' (correctly: Group 1- 100%, Group 2- 80%, P=0.037), , ' Press the MDI and inhale at the same time' (correctly: Group 1- 90%, Group 2- 57%, P=0.030) and 'Hold the breath for 5-10 seconds' (correctly: Group 1- 80%, Group 2- 40%, P=0.043). Conclusion The study has shown that the most patients use their MDI inaccurately. Apart from hospital, which is the most frequent place of education, education is performed also on other levels of healthcare system and by healthcare personnel of different profiles. Education is not standardized and it is not carried out sufficiently enough in order for patients to master and maintain the proper use of aersol pump.
