Sažetak
Propranolol, a first generation nonselective beta blocker, has not been adequately tested in heart failure. Use of beta blockers in general was once absolutely contraindicated in patients with congestive heart failure due to their negative inotropic characteristics. Strong evidence of clinical benefit was shown in large randomized trials with several newer cardioselective beta blockers (bisoprolol, metoprolol, carvedilol, nebivolol) in patients with heart failure. These trials have made beta blockers a standard of care in treating chronic heart failure. Beta blockers do not show a class effect and propranolol is not recommended in heart failure. This chapter explains the rationale for the use of beta blockers in heart failure. It reviews scientific and expert literature dealing with this issue with a focus on trials investigating the efficacy and safety of administrating propranolol in different types of cardiomyopathies and heart failure. Using propranolol in certain conditions such as hypertrophic obstructive cardiomyopathy or co- existing uncontrolable ventricular tachycardias should be considered.
