Sažetak
Purpose: To present and point out the importance of coronal CT as a preoperative stage in FESS (functional endoscopic sinus surgery), because of many anatomical variations in sinonasal region. Patients and methods: 74 patients were hospitalized during the 2005. year on Department of ORL, in Clinical Hospital Center Rijeka, planning FESS (functional endoscopic sinus surgery). Coronal CT was performed preoperatively. Anatomical variations of sinonasal region were analyzed, for optimal planning and safety operation: nasal septum, conha nasale media, ostiomeatal complex, uncinate process, ethmoidal bulla, Haller's cells, lamina papiracea, ethmoidal roof, cribriform plate, dehiscence in the bony covering of the carotid artery. Results are compared with literature. Results: We found 37 (50%) patients with normal anatomical status in sinonasal region, 20 (26.1%) with septal deviation, and concha bullosa in 3 (4.1%) patients. Concave form of nasal concha media was found in 1 (1.3%) patients, and dehiscence of lamina papiracea in 7 (10.4%). Haller's cells were found in 4 (5.5%) patients, dehiscence in the bony covering of the carotid artery in 1 (1.3%). Ager nasicellulae were found in 1 (1.3%) case. Conclusion: Functional endoscopic sinus surgery (FESS) is a minimally invasive technique in which sinus air cells and sinus ostia are opened under direct visualization. As a preoperative procedure, coronal CT is neccessary to be done, because of many anatomical variations in sinonasal region. In comparision with results in literature, frequency of septal deviation in many papers is much higher than it is in our group.
