Sažetak
Twentyfour patients with rheumatoid arthritis with acute onset bave been analysed and compared with 24 patients having rheumatic fever, and with 24 cases of chronic classical rheumatoid arthritis. Nearly ali of the patients with acute onset (20 patients) were checked 1-5 years after the onset of their disease. The number of cases of rheumatoid arthritis with acute onset was 28% of the total number of our patients suffering from rheumatoid arthritis treated in our ward in this period. The patients with rheumatoid arthritis with acute onset were comparatively young persons aged 31 years on an average (16 of the 24 patients were under 40 years of age). There were 13 males and 11 females, as distinet from patients with chronic rheumatoid arthritis, in whom the number of women patients outnumbered the males by two and a half to one. Angina preceded the onset of the disease in only three patients, while in the cases of rheumatic fever it was noticed in 17 patients. In the cases of rheumatoid arthritis with acute onset also the large joints were always involved, similarly as in acute rheumatic fever, with the only difference that in rheumatoid arthritis with acute onset more frequently the small joints of the hands were affected (11 cases, and in rheumatic fever 3 cases). Moderate spleen enlargement was noticed in 3 cases, The temperature ranged up to 38°C in rheumatoid arthritis with acute onset, and even beyond 39°C in rheumatic fever. Regarding laboratory findings it will be of interest to mention that anaemia was found in 17 patients with rheumatoid arthritis with acute onset, in 10 patients with rheumatic fever, and in 11 patients with chronic rheumatoid arthritis. The antistreptolysin titre was raised above 200 u. in 6 patients suffering from rheumatoid arthritis with acute onset, in ali cases of rheumatic fever, and in 2 cases of chronic rheumatoid arthritis. Waaler-Rose titers above 1 : 64 were noted in 10 patients with rheumatoid arthritis with acute onset, in 24 cases of chronic rheumatoid arthritis, and in none of the patients with rheumatic fever. Roentgenologic changes in the bones were found in 6 patients with rheurnatoid arthritis with acute onset, and in 18 cases suffering from chronic rheurnatoid arthritis. Electrocardiographic changes in the heart were revealed in 8 cases of rheurnatoid arthritis with acute onset — 5 cases of moderate disorders of atrioventricular conduction, and 3 cases of distur- bances of interventricular conduction. At the chec&-ups after 1-5 years ali patients that had been marked as rheurnatoid arthritis with acute onset stated having pains in the joints from time to time1 at least, while in 14 patients deformities in the joints were revealed. In one patient signs of ankyIosing spondylitis were found. In the discussion the above data are reviewed and it is concluded that there are no safe signs or tests that would make it possible at the onset of the disease to distinguish between rheumatic fever and rheurnatoid arthritis with acute onset, but that by observing the patient it is possible in a general way to show that the process in the joints in patients with rheurnatoid arthritis rarely subsides, while in patients suffering from rheumatic fever the process disappears completely both subjectively and objectively. The opinion is expressed that rheurnatoid arthritis with acute onset is probably the disease which in the past has been called »polyarthritis rheumatica chronica secundaria«.
