Sažetak
In patients with proximal hypospadias drfect and epecially in patients who were surgically treated more times because of that, it is difficuit to get needed amount of genital skin for reconstruction ofurethral defect. In recent years, after period of initial good results, usingbuccal tissue free transplatant in reconstruction of urethra has a great usage in this area of urogenital reconstructive urology.Here we showed our experience of urethral reconstruction with buccal mucosa grafts free transplant in 11 patients. The mean follow up was 12, 5 months after reconstructive surgery. Patients were divided in two groups. In first group we had 7 patients, who were previously surgically treated three or more times because of hypospadyas, epispadias or urethral stricture. In this group defect was from 4, 5 to 8 cm. Postsurgical complications werw observed in 3 patient. This complications we observed: stricture of anastomosis of urethra, meatus stenosis and urethral fistula.This complications werw corrected after minimal reconstructive surgery. In second group we have patient without previous reconstructive surgery of urethra, or eventually we did in first act only cordectomy. In two patienst in this gropu with penoscrotal transposition urethral defect was from 5 to 7 cm and in two patients with proximal hypospadi was from 4, 5 to 5 cm. In second group we did not observe postsurgical complications. Reconstruction of urethra with buccal mucosa grafts in patienst with primary or secondary proximal urethral defect in our hands has showed as afficent surgical method with good results and minimal morbidity.
