Sažetak
Aims: To compare the parameters of hyperinflation in asthmatic children measured by plethysmographic and diffusion method. Methods: In the prospective study (1996-2002) we followed up 298 children, aged 10.7±3 years, 81 healthy (Control) and 217 children with mild and moderate asthma. 121 were treated with inhaled steroid (fluticasone propionate 100–200 mcg daily), 31 with salbutamol (100 mcg daily as needed) and 65 with ketotifen (2 mg daily). During treatment we measured: residual volume (RV), total lung capacity (TLC) and RV%TLC by 1. plethysmographic (Masterlab Jaeger) and 2. diffusion method (single breath method with carbon monoxide). Results: Plethysmographic measurements of TLC, RV and RV%TLC did not prove the existence of hyperinflation in children with asthma (p>0.05), while diffusion measurements of identical parameters showed significant level of hyperinflation (p<0.001). Conclusion: Diffusion measurements of RV, TLC and RV%TLC showed significantly higher level of sensitivity (S: 80, 51, 88%) and negative predictive value (NPV: 42, 33, 50%) compared to plethysmographic (S: 50, 25, 52% and NPV: 15, 15, 11%). The lung function parameters, measured by diffusion method, give a better insight of asthma diagnosis and control in children.
