Sažetak
We presented a case of 13-years old girl with dyspnea and cyanosis as main symptoms caused by NHL of right main bronchus (anaplastic, large cell, CD 30+). NHL was diagnosed by fiberoptic bronchoscopic biopsy. CT scan of the chest showed narrowed right hemithorax, ipsilateral dislocation of the mediastinum, consolidation and atelectasis of inferior lobus, extreme narrowing of right main bronchus and obstruction at the beginning of intermediary bronchus. At the back wall of right main bronchus near carina intraluminal prominence was presented. It impacted right main bronchus along 2 cm and resulted in almost complete bronchoobstruction of a 5 mm segment. The point of this case report is to present diagnostic, therapeutic and clinical dilemmas (patient with dyspnea mimicking asthma bronchiale, asphyxial crisis caused by hyperinflation and ventilating mechanism) and complications of treatment (bronchooesophageal fistula). Patient was treated by NHL - BFM 95 protocol. Flexible fiberoptic bronchoscopy plays important role in dyspnea clarification and following up of the result of treatment in children with bronchial tumor.
