Sažetak
In this paper we are concerned with the role of conceptual metaphors in discourse dealing with some chronic diseases. Specifically we compare the doctor-doctor and the doctor-patient communication. We start from the premise that metaphors in the former type of communication primarily are more schematic and have a terminological function, while in those genres in which the latter type communication is in the focus we find very different types of metaphors: they are chiefly structural metaphors with fairly elaborate mappings. The function of the metaphor in the latter case is to shed more light on the patients condition by forming a conceptual bridge between the pool of expert knowledge and the lay/popular knowledge by means of exploiting a rich network of mappings. These metaphors have a key role in improving patient compliance. In our corpus consisting of texts dealing with some endocrine disorders, we have found that the two types of metaphors hardly mix, but are rather in a sort of complementary distribution from a synchronic point of view. However, adopting a more dynamic, diachronic perspective, it can be discerned that metaphors may migrate between genres, thus forming stages in their life-cycle: structural metaphors may start out in doctor-doctor communication during initial stages of research in a given area of medical science, to be replaced by a more schematic one once certain insights are gained that form the core of a pool of expert knowledge. The same metaphor, freed of its conceptual job for experts, may find then its way into doctor-patient communication.
