Sažetak
Sensory nerve fiber potentials of the peroneal superficial nerve, tibial nerve and sural nerve of the feet were evoked in healthy subjects and in. patients with polyneuropathy. The results obtained by stimulation, with surface strip electrodes were compared with those obtained with subcutaneous needles. In the group of healthy subjects (N = 41), the sensory nerve conduction velocities and amplitude of nerve potentials were higher, and the duration longer when stimulated subcutaneously The difference between the two stimulating methods was not significant. None of the subjects had the nerve potential missed. We focused on patients with sensorymotor polyneuropathy, especially those with skirt changes. In these patients, surface stimulation was very often unable to evoke the sensory nerve potential. The superficial peroneal nerve was examined in 66 patients ; 47 (71.20%) had no sensory potential evoked by surface stimulation However, after additional subcutaneous stimulation, the normal sensory potential was recorded in 29 of 47 (43.90%), and definitely missing sensory potential in only 18 (27.30%) patients. The tibial nerve sensory fibers were analyzed in 58 patients: 38 (65.51%) had no sensory potential on surface stimulation Additional subcutaneous stimulation evoked sensory potentials in 30 of 38 (51.72%) patients. Definitely missing potentials were recorded in only 8 (13.70%) patients. The distal sural nerve was analyzed in 57 patients: in 48 (55.20%) of them, sensory potentials could not be provoked by surface stimulation In 35 of 48 (40.22%), sensory potentials reappeared on subcutaneous stimulation, while definitely missing potential was found in only 13 (14.90%) patients. Study results indicate that the absence of nerve potential on surface stimulation, is not a reliable indicator of peripheral sensory nerve lesion. The findings thus obtained should therefore be confirmed by subcutaneous stimulation.
