Sažetak
Differences in course and outcome of gestational diabetic (GDM) pregnancies were analyzed in two periods between which changes were made in antepartal management. Longitudinal study involved singleton GDM pregnancies in two 7 yrs periods (1, N=491 i 11, N=473). Following data were observed (period I and 11, respectively): mean gestational age (38.7:t2.5 vs. 38.5ą2.6 wks), prematurity rate (21.2% vs. 17.6%), multiparity rate (60.6% vs. 57.1%) and mean number of previous deliveries (1.7ą1.0 vs. 1.6ą0.9) showed no significant difference between periods. Rate of mothers aged >35 yrs increased (25.9% vs. 35.0%, p<0.05), while rate of stillbirths in women with history of stillbirths decreased (1.8ą0.9 vs. 1.4ą0.6, p<0.05). Rate of urinary tract infection declined from 18.2% to 11.6% (p<0.05). No difference was found in rate of women who required insulin therapy (32.9% vs. 31.4%), rate of neonatal macrosomia (35.8% Vs. 35.2%), disproportionate macrosomia in term neonates (29.2% vs. 34.1%), mean Apgar score (9.5ą1.1 vs. 9.6ą1.1), rate of neonates with Apgar score <7 (4.6% vs. 2.8%), rate of stillbirths among term (1.5% vs. 0.4%) as well as among preterm deliveries (5.6% vs. 3.2%). In spite of increased efforts, current management of GDM pregnancies may have little effect on birth weight, ponderal index and neonatal complications.
