Sažetak
Preeclampsia is the most common disease in human pregnancy and the leading cause of both maternal and perinatal morbidity and mortality. The etiology of preeclampsia is unknown, but several lines of evidence suggest the placenta as the primary locus of malfunction. Extravillous invasive cytotrophoblast from preeclamptic pregnancies has a diminished invasive potential. Shallow invasion of the cytotrophoblast into the placental bed and spiral arteries in preeclampsia, followed by the absence of widening of spiral arteries, causes reduced blood supply to the placenta with ensuing hypoxia of the feto-placental unit and may eventually result in placental and fetal growth retardation. The mechanisms regulating trophoblast invasion in normal pregnancies as well as the distinct sites of dysregulation in preeclampsia are unknown. Abnormal integrin expression has been implicated. The strong stimulative effect of endothelin-1 on the proliferation and invasion of first trimester trophoblast and decreased release of endothelin-1 from trophoblast in preeclampsia suggest the role of ET-1 in the etiology of preeclampsia
