Sažetak
The purpose of this study was to evaluate clinical and radiological results associated with the use of an extra-large uncemented porous-coated component for acetabular revision in the presence of bone loss.Tventy-nine extra- large uncemented hemispherical acetabular compo¬nents were used for revision after aseptic failure of a total hip arthroplasty in thirteen man (14 hips) and fifteen vvomen (mean age at revision sixty-fooryears ; rangeforty- foorto eighty-three years). The analysis of the acetabular defects was done preoperatively according to the AAOS and Paprosky classification. We studied 10 patients with acetabular defectsType 2A, 11 patients Type 2B, 4Type 3A, and 4 patients Type 3B according to Paprosky's classification. According to AAOS- classification 22 patients were classified asType III, 4 patients asType II, and 3 patients as Type I.The revision implatant had an outside diameter of >or 66 mm in men and >or 62 mm in vvomen, or is greater than 10 mm larger than the normal contralateral acetabulum. Mean fol- low-up was 25 months (range 6 - 73). Nighteen patients received morsellized bone allografe, and additional bulk allografts were used in 8 cases. No acetabular component had been revisect and none vvere loose radiographically. Assessment of clinical outcome vvas done on the scale of Merle d'Aubigne'and Postel. Mean score of pain vvas 5, 2.( range 4-6 points), mean score for func- tion 4, 9 (range 3-6 points), and mean score for the range of motion 4, 6 (range 3-6 points). Our results strongly support the use of jumbo uncemented acetabular components in the revision of most loose acetabular components associated with bone loss.
