Sažetak
Plate osteosynthesis is recognized as the treatment of choice for most articular, many metaphyseal and a few diaphyseal fractures. Biological plating techniques are those in vvhich blood supply to the fractured fragments is maximally preserved.The objective of biologic fixation is to assist physiological process of bone healing wiselyand optimally with minimal amount of operative intervention.The development of indirect reduction techniques (Mast et al 1989), the development of vvave plate (Brunner andVVeber 1981) and the bridging plate (Hei.temeyer 1985) brought about a basic change to fracture treatment using plates. Conventional plating techniques if applied to multifragmentary fractures lead to a variety of complications like delayed union or non-union, infection and implant failure.This is because in order to achieve anatomic reduction, wide surgical exposure is necessary and the fracture fragments are stripped of the soft tissue attachments. Minimally invasive plate osteosynthesis (MIPO) is one such method in which percutaneously inserted plate is fixed at a distance proximal and distal to the fracture site through minimal exposure.Advantages cited for MIPO are: I .Simpler technique and easy to master. Learnjng curve short 2 .Improved rates of fracture union. 3. Decreased infection rate 4.Decreased need for bone grafting 5. Decreased incidence of refracture after plate removal .A case report of a patient vvith both lovver le'g fractures, in the area of proximal metaphyses , as a result of a car accident.The patient has been diagnosed vvith an advanced stage of multiple sclerosis and undergone MlPO-method surgery.
