Sažetak
According to the data provided by the World Health Organization, in 2008 there were 12 667 470 patients newly diagnosed with cancer and, should this trend continue, in 2020 there will be over 15 million newly diagnosed patients. In Croatia, every year 20000 people are diagnosed with some type of cancer. 70% of cancer patients experience pain and in advanced-stage cancers 90% of the patients experience pain. Cancer pain can be acute and chronic and it is often a combination of nociceptive and neuropathic pain and in most cases it is also psychological. Multimodal therapy is essential in treating cancer pain. The basic principles of treating cancer pain are based on the World Health Organization recommendations - the use of analgesics on a three step ladder. It is of key importance to assess the strength and the characteristics of the pain and this is performed according to various scales and the most commonly used ones are the visual analogue scale (VAS), a verbal rating scale (VRS) and numerical rating scale (NRS). Step 1 – mild pain includes the use of non-opioid analgesics together with adjuvant analgesics, or without them. Step 2 – mild to moderate pain includes the use of weak opioid analgesics either alone or in combination with step-1 medications. Step 3 – severe pain, includes the use of strong opioid analgesics in combination with step-1 medications. If the patients experience severe and progressive cancer pain from the very beginning, it is recommended to skip step 2 and initially use strong opioids (analgesic lift). Other procedures of treating cancer pain include anesthesiological procedures , nonpharmacological methods of treatment and surgical methods.
