Sažetak
Psoriasis is a systemic chronic immune-inflammatory-mediated disease which affects 2 % population worldwide and 1, 7 % of Croatian population. The main suggested pathogenetic mechanism in psoriasis, inflammatory process, is associated with the development of various comorbidities which have a significant impact on the patient's health and quality of life. Traditional comorbidities include psoriatic arthritis, inflammatory bowel disease and psychiatric disorders. Recently recognized emerging comorbidities include cardiovascular disease and metabolic syndrome. According to the recent public health approach, it is recommended to provide screening for 11 conditions: arthritis, depression, diabetes mellitus, dyslipidemia, hypertension, metabolic syndrome, nonalcoholic steatohepatitis, obesity, inflammatory bowel disease, alcohol consumption and tobacco use. Croatian Psoriasis Association, non-profit organization since 1977 is supporting psoriasis patients. In 2013 it became a part of EUROPSO (European Psoriasis Association) and IFPA (International Federation of Psoriasis Association) and through this collaboration is trying to improve quality of life of affected individuals and also decrease environmental sociological burden usually caused by lack of sense and knowledge. Through various activities, Croatian Psoriasis Association is trying to improve the quality of life of psoriatic patients. In following years we plan to organize screening for 11 already mentioned conditions in collaboration with Institute for Health Services Research in Dermatology and Nursing in Hamburg, Germany. Psoriasis and its comorbidities represent obvious increased cardiovascular risk factors and they must be taken into account by GP and family physicians when estimating treatment goals for the comorbidities. Also, tertiary care specialists need to introduce a more complex treatment approach, including aspects of lifestyle advise and intervention. Finally, effective long-term anti-inflammatory, disease-modifying therapy (systemic therapy and especially biologics) may contribute to reducing patients' cardiovascular risk and improve the life quality and expectancy.
