Sažetak
Acne fuminans is a rare, disfiguring, and disabling disease, chracterized by the sudden onset of a severe, ulcerative acne. Acne fulminans is associated with systemic features - fever and polyarthritis, and laboratory abnormalities – leukocytosis, anemia, elevated erythrocyte sedimentation rate, proteinuria. Response to traditional acne therapy is poor. Systemic treatment is necessary to prevent significant psychological and social impairment in these patients. Acne fulminans need to be promptly and effectively treated. Here we report two cases of male adolescents with acne fulminans. On admission to the Department of Dermatology and Venerology they were febrile with extensive ulcerated and crusted lesions on face and trunk. They also reported musculoskeletal pain. Laboratory findings showed leukocytosis, elevated erythrocyte sedimentation rate and C reactive protein, with normal level of serum lipids. The treatment of these patients has been difficult. The protocol we used was a combination of corticosteroids for four weeks (followed by slow reduction to zero ) and isotretinoin during six month.
