Sažetak
Background and aim: Modulating lymphocyte response to allergen seems to be one of the principal mechanisms of successful immunotherapy. There are no reports in the literature about changes in the CD45RA molecule on the surface of peripheral blood lymphocytes in response to allergen stimulation. The aim of this study was to explore the expression of the CD45RA molecule after allergen stimulation in vivo, prior to and 6 months after allergen-specific rush immunotherapy. Patients: Forty-two patients (28 M, 14 F ; age 15-37) with high levels of Der p - specific and total IgE and clinical symptoms of allergic disease (32 with asthma, 10 with rhinitis) were included. In 31 patients immunotherapy was applied for six months. The remaining 11 patients served as the control group. Methods: Bronchial challenge was done by dosimeter method (output of nebulizer 5.8 mL/breath). The reaction was expressed as the dose which induced a 20 % fall in FEV1 (PD20 FEV1 all). Allergen was Der p Croatian National Standard with a potency of 65 300 BU/ampoule. Phenotypic analyses of lymphocytes were performed by flow cytometry (mAb Becton Dickinson). Immunotherapy was applied by the rush-method. Results: After allergen challenge all patients had an early asthmatic reaction with mean fall of FEV1 28 ą 8 %. In 45 % of treated patients a late asthmatic reaction occurred with a mean fall in PEF of 31 ą 5 %. Specific immunotherapy significantly diminished early and late bronchial reactivity (p=0.001, p=0.003, respectively). Baseline expression of the CD45RA antigen on peripheral blood lymphocytes in patients did not differ from that in healthy controls. Allergen stimulation induced increased expression of CD45RA+ lymphocytes in all patients (p=0.008). In the group of patients receiving immunotherapy CD45RA+ lymphocytes did not increase significantly after allergen stimulation (p=0.1) when compared to the untreated control group. Conclusions: Antigen stimulation induced a detectable change in the expression of the CD45RA molecule on peripheral blood lymphocytes two days after the allergen-antibody reaction. Specific immunotherapy inhibited the expression of the CD45RA molecule after allergen challenge. Modulated expression of the CD45RA molecule could interfere with activation of lymphocytes and their response to allergen.
