Sažetak
It has long been proved that liver cirrhosis is an insulin-resistant condition. However, insulin resistance is not the only malfunction in glucoregulation observed in these patients. The aim of our study was to determine the effect of various hormones taking part in glucoregulation on the development of glucose intolerance and/or diabetes mellitus in patients with liver cirrhosis. Therefore, we studied the concentrations of glucose and glucoregulatory hormones both in the basal state and during i.v. GTT in four groups of subjects: subjects with non-insulin dependent diabetes mellitus (NIDDM), subjects with liver cirrhosis, subjects with liver cirrhosis and diabetes mellitus, and healthy volunteers. The diagnosis of NIDDM was made according to WHO criteria, and that of liver cirrhosis was confirmed by biopsy. Results showed statistically significantly higher values of immunoreactive insulin (IRI) in the subjects with liver cirrhosis compared to those with liver cirrhosis and diabetes mellitus, in whom a delayed maximal IRI response to glucagon stimulation was also recorded. In the same group, the highest values of immunoreactive glucagon (IRG) were obtained (c=85.49 ; p<0.005), while differences in the values of C-peptide, growth hormone and cortisol between the groups were not statistically significant. The role of glucagon in the pathogenesis of impaired glucoregulation and/or diabetes mellitus in liver cirrhosis is a subject of our present study.
