Sažetak
The effect of ACE inhibitor enalapril on urinary albumin excretion (UAE) was studied in patients with incipient diabetic nephropathy. Eight adult, normotensive patients with insulindependent diabetes, who did not take any medicines except insulin, were monitored for UAE, blood pressure, glomerular filtration rate (GFR), renal plasma flow (RPF) and filtration fraction and immediately before commencing the treatment ; the other two times it was measured after a 4-week treatment with enalapril and four weeks after the treatment was stopped. The therapeutic dose was 20 mg/day. UAE fell significantly in all patients (from baseline 59.3±22.3 mg/24 h to 30.0±14.8 mg/24h) (p<0.01) after a 4-week treatment with enalapril. Baseline GFR (138±35.8 ml/min/1.73 m2) fell slightly to 123.4±17.3 ml/min/1.73 m2 (p>0.1). Conse quently, RPF and FF did not change significantly. Immediately after the treatment, systolic and diastolic blood pressure fell significantly, but they remained within the range of the 95th percentile for their age. Our results contribute to the findings which demonstrate that enalapril can reduce UAE significantly in normotensive diabetic patients with microalbuminuria, and can retard the development of diabetic nephropathy into end- stage renal failure.
