Sažetak
Obesity is an independent predictor of cardiovascular mortality and morbidity in the general population. Recent reports suggest that end-stage renal disease (ESRD) patients with greater body mass index (BMI), on the maintenance haemodialysis (HD), have superior outcomes. This paradox has been referred to as ‘‘reverse epidemiology”. About 30-70% patients on HD have mild to moderate malnutrition. It is very important to assess the nutritional status accurately. Body fat mass (BFM) percentage estimation is an important part the assessment of nutritional status. The aim of our analysis was to establish BFM of the HD patients using caliper and multifrequency bioelectrical impedance spectroscopy (BIS), and to compare results. In this cross-sectional study we enrolled 137 patients (78 female and 59 male) mean age was 66.4±13.3 years that were monitored over a period of theree months. All patients were dialyzed four hours, three times a week, on biocompatible synthetic membranes, for at least six months. Spent time on HD was 46.1±44.4 months. BFM was measured using a Body Composition Monitor (BCM - Body Composition Monitor, Fresenius Medical Care, Bad Homburg, Germany), and caliper, by experienced medical nurse. Patients were examined two hours after midweek HD season. The mean body mass index (BMI) was 25.2±4.1 kg/m2. The mean of BFM by BIS method was 24.65±7.6%, while the mean BFM by caliper was 21±5.7%. After comparison of two methods, we have found that BFM assessed by BIS method had positive correlation with BMI (r=0.60 ; p<0.001). BFM assessed by caliper also showed positive correlation with BMI (r=0.38 ; p <0.001). The assessments of BFM by caliper and BIS were equally good. Our results imply that use of caliper for the assessment of BFM in HD patients could be used in everyday practice because of its efficiency, simplicity, and low cost.
