Sažetak
The calcaneus is the most commonly fractured tarsal bone, and participates in 2% of all fractures. Advances in spiral CT and postprocessing imaging possibilities ensures better diagnostic evaluation of the fracture with better access for the therapeutic approach. The aim of this study is to show the possibilities of spiral CT in diagnosing the calcaneus fracture, the extent and the type of the fracture. After plain radiographs and in some cases special projections, spiral CT is performed. Using different 3D methods, all structures close to and as well as the calcaneus were available for analysis. Multiplanar reformatted images with volume rendering techniques for bone changes and relationships between adjacent structures, together with ankle tendons changes are analyzed. Shaded surface display is used when a disarticulated view is desired. 15 patients underwent spiral CT of calcaneus, for better identification and characerisation of the fracture. CT of calcaneus was performed on single slice CT. Standard protocol is used: 1mm slice thickness, pitch 1. The volumetric data sets are evaluated at an additional workstation. No contrast medium is used. In 11 cases we have found intraarticular and in the remaining patients, extraarticular frcature was diagnosed. Conclusion:spiral CT of calcaneus allows better detection of the fracture pathology and is essential for radiologist to guide the treating physician. Spiral CT significantly improves the functional outcome in the treatment of calcaneus fractures.
