Sažetak
Advanced age is a poor prognostic factor for patients with acute leukaemia. More than 50% of all patients with acute myeloid leukaemia are over 60 years of age, and about 30% of patients with acute lymphoblastic leukaemia are elderly. Complete remission can be achieved in 46 to 67% of patients with pancytopenic mortality of 10 to 25% by the use of intensive chemotherapy, as in younger patients. The lowering of cytostatic doses results in a decreased percentage of complete remissions. When deciding on the choice of therapy, it is obligatory to consider the general performance status of the patient, as well as the associated diseases, especially cardiomyopathy due to the toxic effect of cytostatics. Intensive chemotherapy has to be accompanied by appropriate supportive treatments, the most important ones being infection prevention and blood transfusion. New drugs and new formulations of existing drugs, cytoprotectors and myeloid growth factors may improve the outcome for this group of patients. Stem cells transplantation with some modifications can be performed even in the elderly patients with a good performance status. BAVC myeloablative chemotherapy can be applied with a low percentage of toxic deaths. Allogenic stem cells transplantation with nonmyeloablative chemotherapy (mini transplant) is feasible in the elderly patients with acute leukaemia. Intensive chemotherapy should be used whenever possible. Additional randomised multicentric studies are needed to identify the optimal method of treatment of acute leukaemia in elderly patients.
