Sažetak
Distilled water nasal challenge significantly increases nasal resistance (NAR) probably due to mastocyte activation in patients with allergic and non-allergic rhinitis. Insignificant correlation between NAR increase and tryptase (try) levels in nasal lavage prior to distilled water challenge was (r= -0.31) was demontrated in our previous research. A group of 15 non treated hyperreactive patients (8 allergic, 7 non-allergic) were subjected to nasal challenge by inhalation of 10 ccm of distilled water. Active anterior rhinomanometry (rhinomanometer PC200, Atmos, Lenzkirch, BRD) was performed before and after the challenge. Nasal lavage with 0.3 ccm of saline per nostril was performed after the last rhinomanometry and try levels in lavage samples determined using standard RIA kits (Pharmacia, Uppsala, Sweden). Significant increase in unilateral nasal resistance after the provocation was demonstrated on the side with lower resistance prior to provocation (p<0.05). On the side with higher basal nasal resistance decreased insignificantly. Total nasal resistance did not increase significantly. There is no significant difference between tryptase levels on more and less patent sides of the nose prior to provocation, but tryptase levels are significantly higher in allergic patients. In patients with low tryptase, response to provocation is not related to the tryptase level, but in the group with high nasal tryptase, response is higher on the side with higher tryptase. Correlation between postchallenge nasal tryptase and baseline resistance on the less patent side prechallenge is highly significant (r=0.89, p<0.05), but the correlation between postchallenge resistance on the worse side and try is insignificant), as well as the correlations of between pre/postchallenge resistances and tryptase on better patent side.. Postchallenge tryptase significantly correlates with prechallenge nasal resistance only on less patent side prior to provocation. In patients with higher postchallenge tryptase levels in nasal lavages, response to provocation is related to mast cell activation. In patients with low nasal tryptase, response to challenge is probably mediated by some other mechanism.
