Sažetak
Chronic levodopa treatment in PD is often complicated by halucinations and delusions. Clozapine, an atypical neuroleptic, possesses a relatively high affinity for the dopamine D4 receptors. It has been reported that clozapine treatment improves psychosis in PD patients. However, contradictory resuzlts have been reported about CBC changes during cloizapine administration. Here we report nine PD patients (age range 57-73 years, disease duration 5-13 years, H&Y stage 2-4) who developed hallucinations and delusions. Patients were commenced on 25 mg clozapine (Leponex) at night and the dose titrated to a maximum of 75 mg until the symptoms were abolished. Our preliminary open study indicated that clozapine shouls be considered for the treatment of psychosis in selected PD patients.
