The aim of our study was to compare characteristics, clinical course, and outcome of treatment in 32 (2.4%) children from our database of 1358 patients (pts) with differentiated thyroid cancer (DTC). All pts were operated on between 1971 and 2011 with subsequent follow-up in our institution. There were 30 children with papillary thyroid cancer (PTC), and 2 children with follicular thyroid cancer (FTC). Mean follow-up time was 10 years (range 2–32 y). Median age at the time of diagnosis was 14.5 years (range 6 to 18 y) ; f:m ratio was 5:1. Median tumor size was 20 mm (range 4 to 50 mm). Locoregional metastases were found during the initial operation in 17 pts (53%). At presentation distant metastases were detected in 4 (13%) pts with PTC. Median tumor size in these pts was 39 mm. 4 PTC pts with advanced disease received an average of 24.6 GBq (665 mCi) of I- 131. Locoregional recurrence developed in 8 pts with PTC during follow-up and distant metastases developed in 2 pts with PTC. Serum thyroglobulin (Tg) level ranged from 57 to >600 ng/mL in PTC pts with distant metastases. Locoregional metastases were detected by neck ultrasonography (US) and confirmed by US-guided FNAB with cytology. After therapy, four PTC pts with distant metastases were disease free, one was stable and 1 pt was lost to follow-up. No patient died. In conclusions, DTC is rare in children. It is associated with high incidence of distant and locoregional metastases. Results in this study confirm that children with DTC have favorable outcome, even when they have advanced disease.
