Sažetak
The surgical approach to liver echinococcosis is still a controversial issue. This study shows our results of surgical treatment of liver hydatid cysts during a 5-year period. METHODS: A prospective study of 20 patients operated on in a 5-year period (2002-2006), in the Department of abdominal surgery at the University hospital Dubrava, Zagreb, Croatia with hepatic hydatid cyst. All patients were pre- operatively treated with dalben. We have performed 4 different operative techniques as follows: Open total pericistectomy (TP) - 10 patients Papadimitriou, open partial pericistectomy) - 6 patients Laparoscopic pericistectomy (LP) - 2 patients Laparoscopic partial pericistectomy (LPP) - 2 patients RESULTS: Median operative time for OP was 153 minutes (range 120.0 – 180.0) and for Papadimitriou 103 minutes (range 60.0 – 180.0). Median operative time for LP was 107 (range 90.0 – 140.0) and for LPP 90 minutes. Patohistological analysis has showed that 15 % of patients had simplex cysts instead of hydatid. Ultrasound had 90 % of diagnostic certainty, CT 85 % and serological methods 75 %. CONCLUSION: Although our number of patients was not very large, we suggest that a certain number of patients receive unnecessary conservative therapy, which is very expensive and unpleasant for the patient (one month of hospitalization) so we would suggest further researches on this subject.
