Sažetak
In this report, we describe the case of a 35 year old man, who
presented at the emergency with a complaint of long-lasting pain
(lasting 10 years) in the femoral region. Our aim was to show an unusual chronic complication of osteochondroma, a hematoma (which can appear like a tumorous mass on radiological examination) in the patient.
X – Ray, CT and MRI were all used for during the follow up of this patient.
X-Ray examination showed the presence of an osteochondroma (exostosis)
with dimensions of 6x4cm in left femoral neck.
Soft tissues showed no pathological changes however MRI was recommended.
MRI showed a heterogeneous mass in the left femoral neck. To confirm if this
mass was indeed a tumor or was due to a long standing hematoma CT
angiography was performed. This showed that there is no pathological vascularization of the mass.
The Hematoma was surgically removed and diagnosis was confirmed using
pathohistology. Osteochondromas or osteocartilaginous exostoses are common benign bone
tumors. Most commonly, osteochondromas arise in the metaphyseal regions of
long bones (femur, humerus, and tibia).
Normally these osteochondromas invoke mechanical irritation that leads to
bursa formation, snapping of the overlying tissue even winging of the
scapula, but in our case it was mechanical irritation, which lead to
the formation of a long
lasting hematoma giving rise to an unusual MRI appearance.
There is also a rare condition known as Hereditary multiple exostoses (HME), in
which multiple osteochondromas appear during childhood.
Many authors have reported open surgical excision when these
osteochondromas become symptomatic otherwise a non-operative treatment is to
observe the patients because most lesions are asymptomatic. Lesions found
incidentally can be observed, and the patient can be reassured.
