Sažetak
Objective: Although carbamazepin is relatively well tolerated, its rarest but most serious adverse effects, among others, are skin reactions. Clinical pictures of skin changes may alternate up to life-threatening dermatological syndromes like the Stevens-Johnson syndrome, where fatal outcomes may occur in up to 30% of the cases. Methods: A female patient was admitted to our Department of Psychiatry. The reason for admission was the worsening of her mental status in terms of present persecutory delusions and ideas of references. Atypical antipsychotic was introduced at first, and later the treatment with carbamazepin started. Four days after the initial dose of carbamazepin, skin alterations in the form of erythematic changes in area of the thorax, abdomen, groin, gluteus and upper leg, were develop. Results: Immediately after noticing skin changes the use of carbamazepin was stopped. After eight days since the beginning of the rash, of excluding carbamazepin and by using dermatological therapy, local skin exam showed complete resolution of the changes. Conclusion: According to the fact that skin changes developed soon after the introduction of carbamazepin, normality of laboratory results obtained after noticing it and since nothing else was changed in pharmacotherapy, we consider that the described skin changes are adverse events on carbamazepin. Dermatological therapy primarily was referring to “covering” possible diagnosis with such presentation.
