Orofacial pain (OFP) is the presenting symptom of a wide range of diseases which also may occur in the absence of other clinical signs or detectable scanning tests or laboratory abnormalities. The most common clinical entity in our sample was non-odonthogenic trigeminal neuralgia (TN), disorder of the fifth cranial nerve characterized by attacks of intense, stabbing pain affecting one side of the face. The exact cause of trigeminal neuralgia is still not fully understood. Success of the treatment depends on its early identification and understanding of the underlying mechanisms. The aim of the study is to analyze the quality of chronic neuropathic OFP and sensory alterations in middle-aged women in correlation with pain intensity and the duration of symptoms. A total of 33 women aged from 40-60 with chronic OFP with duration of symptoms ranging from 7 to 120 months and 30 pain-free age- and gender-matched controls were examined. A standardized diagnostic protocol was applied to all of them, consisting of the following: 1) demographic data ; 2) evaluation of presence of neuropathic pain measured by The Leeds Assessment of Neuropathic Symptoms and Signs Pain Scale (LANSS) ; 3) estimation of overall pain on the visual analog scale (VAS) at the time of evaluation and during the previous month. Following the seven item LANSS we found dysaesthesia in 96.96 %, autonomic dysfunction in 69.69 %, evoked pain in 81.82 %, paroxysmal pain in 93.93 %, thermal sensation in 69.69 %, presence of allodynia in 42.42 % and altered pin-prick threshold in 60.60 % of patients. Average VAS at the moment of examination was 7.21, and during the previous month 7.27. Our preliminary results show high prevalence of alterations in the perception of pain quality measured by LANSS in middle-aged women suffering from chronic neuropathic nonodontogenic OFP. Results were similar in the age group between 40-45 years, as well as that of 55-60 years.The combination of seven LANSS items is individual and does not correlate to age, pain intensity or length of the symptoms.
