Sažetak
Introduction: The role of ANP in congestive heart failure is well known, but there is not much data about its role in cardiac dysfunction caused by severe sepsis. Methods: 9 critically ill patients who for diagnosis of sepsis were included in this study. All these patients had pulmonary artery catheter inserted, either Oximetrix, or Opti-Q (Abbott-Critical Care System, Abbott Laboratories, North Chicago, IL, USA). We compared hemodynamic variables from 87 measurements with pro ANP plasma levels drawn from right atria. Blood samples were taken parallel with hemodynamic measurements. proANP values were obtained by sendwich enzyme immunoassay by Biomedica Gruppe, Wien. Results: 1. There is significant difference in proANP values in patients who developed ARDS and in those who did not. In the group which did not fullfill criteria for diagnosis of ARDS (n=4), proANP mean valuewas 2180ą780 fmol/ml. In the ARDS group, the proANP value was 4090ą1250 fmol/ml. Statistical difference was tested by the Kruskall-Wallis test and is highly significant: p<10-7. 2. We did not find significant correlation of proANP with any other hemodynamic variable obtaine from pulmonary artery catheter. Conclusion: proANP seems to be important in the pathophysiology of ARDS.
