Sažetak
Elevated serum creatinine is the usual marker of impaired renal function. Testing the clearance of Technetium DTPA allows the glomerular filtration rate (GFR) measuring. The low molecular weight protein cystatin C produced by cells within body and eliminated by glomerular filtration has been suggested as an indicator of GFR.In the group of 81 children (aged 0.04 to 14 years), cystatin C, creatinine and GFR by DTPA clearance were measured. DAKO Cystatin C kit was used for determination of cystatin C on an Olympus AU 600 analyzer. GFR ranged from 60-145 ml/min. The cystatin C and creatinine values significantly correlated with GFR rcysC=0.47 and rcreat=0.59, respectively (p<0.001). There was no correlation between cystatin C and age, while creatinine significantly correlated with age (r=0.46, p<0.001). Diagnostic accuracy in the identification of reduced GFR, measured as area under the ROC plot, was greater for cystatin C than for creatinine (0.66 and 0.59, respectively). In contrast to creatinine, serum cystatin C reflects renal function in children independently of age. The results suggest that cystatin C is a reliable marker of an early stage of impairment, and can be recommended for renal function testing in children.
