Sažetak
AIM: To investigate whether tumor position within thoracic wall and its relation to visceral pleura impact on release of soluble fragment of cytokeratin 19 (CYFRA 21-1) in blood. METHODS: The retrospective analysis includes 445 subjects ; 347 non-small cell lung cancer (NSCLC) patients, 79 lung metastasis and 19 benign cysts. The blood samples were taken before surgical treatment. The computed tomography was performed using the instrument series SCT-7800T by Shimadzu Corporation (Japan) in Clinical Department of Diagnostic and Interventional Radiology, University Hospital Centre Zagreb. The determination of blood levels of CYFRA 21-1 has been done on fully automated instrument Elecsys 2010 (Roche Diagnostics, Switzerland) in Clinical Unit for Molecular Biology and Genetics of Tumors, Clinic for Pulmonary Diseases Jordanovac, University Hospital Centre Zagreb. RESULTS: Based on CYFRA 21-1 levels there are significant differences between three subgroups of NSCLC ; tumors with central position which show the highest levels, peripheral and intermediary (p=0, 0028). There are no significant differences in lung metastasis or cyst group. The patients with pleural retraction exhibit the highest values. CYFRA 21-1 shows significant differences in NSCLC group between pleural tail, pleural thickening, retraction and normal relation to pleura (p=0, 0023). In comparison of all the subjects with normal relation to pleura, there are significant differences between NSCLC, metastasis and cysts with highest values in NSCLC group (p=0, 0001). CONCLUSION: The central position involving proximity or invasion of bronchial tree, pleural thickening or retraction, is accompanied with the high concentration of soluble fragment of cytokeratin 19 in blood in NSCLC. Taken together radiomorphologic features and tumor marker measurement can serve as prognostic parameters as they indicate greater invasiveness of tumor.
