Sažetak
Introduction: Malignant arrhythmias are not very common in non-ST elevation acute coronary syndrome (NSTE-ACS), and when occur it represent very high risk that require urgent invasive approach. Sometimes, it is difficult to explain malignant arrhythmia if not all elements of the acute coronary syndrome are present. Case report: We present a case of a 70-year old woman with known LBBB and previous ostial right coronary artery stenting who suffered cardiorespiratory arrest due to primary ventricular fibrillation. There was only a slight elevation of troponin-I, and there were no significant regional motion wall abnormalities. After successful resuscitation and stabilization in a smaller regional hospital she was transferred to our institution where coronary angiography was performed. Subtotal ostial in-stent restenosis was found, that was successfully stented with everolimus eluting stent. There were no recurrent arrhythmias, nor recurrent myocardial ischemia. Strategies for malignant arrhytmia management and ostial coronary artery in-stent restenosis are discussed.
